Prison staff discovered that Cuffy, aged 44, had managed to deceive them by placing a dummy in his bed, creating the illusion that he was sleeping in his own cell. This ruse allowed him to slip away unnoticed and spend up to 16 hours in the cell of another male Category A prisoner. Cuffy was convicted in 2003 for the brutal slaying of Sandy, whom he lured to his home in Usher Green, Lincoln. After she visited seeking a friend, Cuffy, then 35, plied her with alcohol before taking her to Boultham Park in Lincoln. There, he stripped her naked and attempted to assault her, ultimately slashing her throat with a Gurkha knife. He returned to Derby, his hometown, where he registered with a general practitioner and was referred to a psychiatrist. However, no comprehensive risk assessment was conducted, and he was released back into the community without supervision. Following the publication of the report, Sandy’s mother, Jackie James-Clarke, expressed her shock and frustration. The 36-year-old woman stated she was “dumbfounded” that a sex offender could change his name and go unchecked, emphasizing that the failure to communicate between agencies was a key factor in her daughter’s death. She called for stricter measures to prevent such cases from recurring, stating, “Let us hope that Sandy’s death will not be in vain. This man will continue to be a danger forever, and I hope that he will never be released from prison.”
