    In a detailed and sobering report released today, the tragic death of Belper toddler Riley Pettipierre has been scrutinized, revealing a series of systemic failures and communication breakdowns among health and social care professionals involved in his case. The report, which is the result of a Serious Case Review, highlights numerous shortcomings that contributed to the preventable death of the young child.

    One of the most alarming findings was that health staff failed to inform social workers about the fact that Riley’s mother, Sally Dent, was a known drug addict. This critical piece of information was never relayed to the appropriate authorities, despite the evident risks posed to Riley. The report underscores that this lapse was part of a broader pattern of poor information sharing between health professionals, which significantly hampered efforts to safeguard the child.

    Furthermore, the review identified deficiencies in record-keeping and the lack of effective systems to monitor and respond to the risks associated with drug abuse. It was noted that drug treatment workers involved with Sally Dent did not prioritize Riley’s safety over their client’s needs, nor did they conduct thorough assessments to determine whether the toddler was at risk. The failure to adhere to child protection procedures was evident, as several professionals did not refer the family to Children’s Social Care, despite clear indications that intervention was necessary.

    Riley’s death occurred at his home on Kilbourne Road, Belper, on March 13, 2012. The child tragically died after ingesting methadone that his mother had placed in a child's beaker for her own use during the night. Shaun Binfield, also known as Sally Dent’s partner, was convicted of placing the beaker in a location accessible to Riley, which directly contributed to his death. Both Sally Dent and Shaun Binfield were later convicted in February of manslaughter, with Dent also found guilty of cruelty to a child. Evidence presented during the trial revealed that during the final months of Riley’s life, he had ingested a mixture of drugs including cocaine, heroin, and methadone, substances that Dent herself used, often combining crack cocaine and heroin with methadone.

    In response to the findings, Lynn Woods, the chief nurse and director of quality at NHS Southern Derbyshire Clinical Commissioning Group, stated that no individual had lost their job as a result of these failures. She emphasized that most issues stemmed from systemic problems rather than individual negligence. Ms. Woods clarified that a Serious Case Review is intended as a reflective process to learn lessons, not as a disciplinary procedure, and that disciplinary actions would be at the discretion of the individual organizations involved.

    The report concluded that had Riley been referred to Children’s Social Care earlier, his death might have been prevented. It was also revealed that, despite the involvement of multiple health professionals and drug workers in Sally Dent’s care, Riley’s birth in November 2009 was never reported to children’s social services. This omission was particularly concerning given Dent’s known history of drug abuse.

    When the child was born, he exhibited symptoms consistent with drug withdrawal, yet proper procedures were not followed. The midwife involved failed to request a meeting with Children’s Social Care or the health visitor, which could have prompted necessary safeguarding actions. After Riley’s birth, health professionals involved in Dent’s drug treatment did not effectively communicate or compare information about her drug use, leading to conflicting reports that went unaddressed. The report suggests that there may have been a culture at the drug treatment clinic that discouraged proactive safeguarding referrals, as no referrals had been made in the previous 12 months.

    Despite these failures, the report noted that Riley appeared to be thriving at times, and a positive bond had developed between him and his mother. The living arrangement was described as supportive, which added complexity to the case. Ms. Woods acknowledged that there was no evidence of a systemic culture among drug workers to avoid social care referrals but admitted that in this case, health professionals should have made such a referral.

    During the final ten days of Riley’s life, police received information linking Sally Dent and Shaun Binfield to drug dealing activities. However, authorities failed to connect the dots, and the couple was not identified as parents of a small child living in their home. The report criticizes this oversight and states that police have since been instructed to inform the Central Referral Unit, which includes social care staff, whenever they are involved in incidents that could impact a child's safety, aiming to prevent similar tragedies in the future.
