SHEFFIELD MAN IN MENTAL HEALTH CARE FAILURE BLAMED FOR PARENTS' STABBING DEATHS

A coronial investigation has flagged serious issues relating to inter-agency communication in the case of a man who murdered his parents in Totley.

Bryan and Mary Andrews were killed by their son, James Andrews, on November 27, 2022.

The defendant was later sentenced for manslaughter at Sheffield Crown Court in July of the previous year and was held at Rampton Hospital under section 37 of the Mental Health Act 1983 for ongoing treatment.

Evidence at the trial revealed that Andrews had stabbed his father nine times and his mother 82 times.

On the day of the killings, Andrews told police, "I've just killed my mum and dad," attributing his actions to hearing voices and believing that a divine force was making him do it.

He had been experiencing these voices for 14 months prior.

Earlier responses to his mental health issues included reports from his sister and a referral to mental health services; however, he did not qualify for intensive intervention.

It was also recorded that Andrews was found by police after the incident, declaring, "I've just killed my mum and dad." Witnesses reported that Andrews was agitated and was seen with a knife wrapped in a sheet shortly before the murders.

His sister criticized the mental health system for failing her brother at a critical time, stating, "My brother needed help and I feel the system let him down when he was in most need." The coroner’s Prevention of Future Deaths report pointed to failures in communication and procedures among health and social services, including delays and refusals of referrals, despite evidence of psychosis and suicidal tendencies.

Specifically, on April 29, 2022, Andrews contacted a crisis team but was referred elsewhere without involving his neurologist.

Records also indicated lapses in medication review follow-ups and referral processes, even with documented concerns related to his mental health.

The report warned that if such systemic issues are not addressed, similar tragedies could recur, highlighting the importance of better managing mental health risks like those demonstrated in Andrews' case.
